Free Application for Federal Student Aid (FAFSA): Students should complete the Free Application for Federal Student Aid to apply for federal financial assistance.

Satisfactory Academic Progress (SAP) Appeal: Students can submit an SAP appeal to the Office of Financial Aid explaining their mitigating circumstances, to request reinstatement of financial aid.

Scholarship Consent to Release Form: We can only share details about your financial aid with a scholarship or external organization if you give us written permission.

Verification Statement of Identity (V4): The U.S. Department of Education (ED) created the Verification process to confirm the accuracy of information provided on the FAFSA. This verification form is required to verify your identity, and it requires the student's signature, which must be signed in the presence of a notary, or in person at the financial aid office.

Print and fill out the Verification Statement of Identity form, then submit it to our office using one of these two options:

  1. Once notarized, mail the original to the following address:

  1. Deliver the completed form with proof of identification to the Spelman Financial Aid Office.  We are in Packard Hall, on the 1st floor.
