COVID-19 Vaccination Requirements

The Centers for Disease Control and Prevention (CDC) recommends that everyone 12 years and older be fully vaccinated and boostered to protect against the virus and other variants.

Vaccination cardVaccinated faculty, staff and students are required to electronically submit a copy of their official COVID-19 vaccine card, including the booster, via Etrieve.

  • Sign in to Etrieve
  • Click on the “Return to Campus Documentation” link under “General”.
  • Select the correct Submission Type (i.e., “COVID-19 Proof of Booster”, etc.).

In general, people are considered fully vaccinated as follows: two weeks after their second dose in a two-dose series such as the Pfizer or Moderna vaccines, or two weeks after a single-dose vaccine, such as Johnson & Johnson's vaccine.

For the Pfizer or Moderna vaccine, you are eligible for a booster after 5 months of vaccination series completion. For the Johnson & Johnson (Janssen) vaccine, you are eligible for a booster after 2 months of vaccination series completion.
