Vaccinated faculty, staff and students are required to electronically submit a copy of their official COVID-19 vaccine card, including the booster, via Etrieve.
- Sign in to Etrieve
- Click on the “Return to Campus Documentation” link under “General”.
- Select the correct Submission Type (i.e., “COVID-19 Proof of Booster”, etc.).
In general, people are considered fully vaccinated as follows: two weeks after their second dose in a two-dose series such as the Pfizer or Moderna vaccines, or two weeks after a single-dose vaccine, such as Johnson & Johnson's vaccine.
For the Pfizer or Moderna vaccine, you are eligible for a booster after 5 months of vaccination series completion. For the Johnson & Johnson (Janssen) vaccine, you are eligible for a booster after 2 months of vaccination series completion.
