Data Request Form

If you need data outside of what is available in existing resources, please submit your request using the form below. Data request completion time varies based on the data that is requested. Most request are completed within 10 business days. Please allow a minimum of 48 hours for completion.

Please be sure to upload any relevant documentation that is necessary to complete your request, and click the Submit button upon completion.

Questions to Consider

1. Did I specify the population being requested (e.g., specific majors, total student population, faculty, staff, etc.)?
2. Have I specified the semester(s) or year(s) being requested?
3. Do I need a total count (summary) or an individual list of variables?
4. If I am requesting a list of students or graduates, what variables do I need (e.g., student ID numbers, gender, race, age, residency, major, classification, etc.)?
5. Do I need to attach any files to help clarify the data I am requesting?

Fields marked with an asterisk (*) are required.

Examples:

• "What was the 2^nd and 3^rd YR retention rates of Economic majors from Fall 2011 - Fall 2015?"
• "How many students over the age of 25 were admitted and enrolled from Spring 2010- Fall 2019?"
• "Our department is assessing its impact on incoming FTFY and wants to know the number of students who entered as Undeclared and later majored in English from Spring 2010- Spring 2021."
