Please complete the following forms: (All forms must be typed)

• Spelman College Biographical Information/Acceptance of Risk
• Insurance Form
• Medical History
• Medical Release Waiver

All scholar-athletes will be required to provide the following documents in addition to forms:

• Copy of Insurance card(s) front and back
• Cashiers Check or money order (make Payable to Spelman College) in the amount of $65.00. Please indicate your sport on the “for” line of this form of currency.

Scholar-athletes who choose to have their physicals performed by their own physicians must provide a Physical Form.
